Please complete all fields and upload the requested documents (in PDF or JPG format) to create a new Facilities Schedules™ Software user account and related Group(s).

Class Description
Class 1 Students/School District Sponsored Groups/Activities, Community Organizations/School Benefactors, Township Government
Class 2 Residents/Non-revenue (501c3 will be needed.) You can upload this in your Group profile upon logging in. Here are some instructions.
Class 3 Semi-profit/For Profit Community Organizations/Businesses, Non-Residents/Outside Organizations

Group Information

If you are an internal staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name.

  Enter your group's city

  Select your group's state

  Enter your group's zip code

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

Upload your insurance certificate (PDF or JPG)

Enter expiration date in MM/DD/YYYY format
